With this plan, you eliminate monthly payments and only pay when you have dental work done. There is no hassle and no waiting periods or insurance claims to worry about. Simply pay one low annual fee and you’ll receive a discount on your dental work, free x-rays, exams, dental cleanings, eye exam, retinal photos, 25% off contact lens exam, and 20% discount on all lenses, and frames.

What Do I Get?

With one low annual fee you’ll receive:
Dental Benefits:

- Initial Comprehensive Exam
- Periodic Exam
- Limited exam (emergency exam)
- Perio Maintenance (3 per year)
- Basic X-rays (with initial exam)
- Panoramic x-ray
- Any additional PA’s needed
- Fluoride Treatment
- Oral Cancer Screening
- Intraoral/Extraoral Photographs
- Oral Hygiene Instructions
- PLUS 20% discount on all in-office procedures

Eye Benefits:

- Eye Exam

- 25% off contact lens exam

Note: Up to two cleanings a year; Regular cleanings are for patients with healthy gum tissue, in absence of any bone loss.  

*Patients who are diagnosed with periodontal disease will receive a “deep cleaning” or Scaling & Root planing at 20% OFF.

* Basic X-rays include 4 Bitewings and 2 Periapical X-rays and Panoramic X-rays

DENTAL MEMBERSHIP PLAN TERMS AND CONDITIONS:
- Family members cannot be substituted in for another family member.  Additional family members can be added at any time.
- Your effective date is the day you sign up and your renewal date is the same date every year
- Cleanings must be performed before the end coverage date. Regular cleanings are for patients with healthy gum tissue. Patients with periodontal disease will receive deep cleaning at 20% off.
- This is a dental discount plan and is NOT dental insurance. It cannot be combined with any other dental insurance or any other offers.
- This is a vision discount plan and NOT vision insurance. It cannot be combined with any other vision insurance or other offers
- Payment is due at time of service for any work received.
- Rates are subject to change annually.
- Membership dues are not refundable.
- An additional fee may be charged for any missed, cancelled or broken appointment without 24-48 hours prior notice.
- Membership is good only at Fusion Eye & Dental Care, therefore if you are referred to a Specialist, discounts offered to members will not apply.
